I would still use the existing trunk line as the new leader, and when you get a new shoot towards the top wire that up as the leader

Have you poked around at the nebari yet?
For me the roots would dictate the new leader on this one if it were mine.

I've had a little poke around the base, nothing to invasive though. The roots I found aren't very big, so there must be a lot happening under the soil. And here's an update for this Mel. Not real work done over the last 2 years and has suffered a little from the neglect, so I chucked in some fresh soil and a dump of fertiliser.
